Center Elders Independence - Eastmont Center & Josie Barrows Center, Eastmont Mall, Oakland
With sites in East Oakland, Central Oakland and Berkeley, our Program of All-inclusive Care for the Elderly (PACE) serves the very impaired and frail elderly who meet Medi-Cal eligibility criteria for nursing home-level care. Funded by Medi-Cal and Medicare, the PACE model of care assumes full financial responsibility for the health and health-related care needs of all enrolled participants. CEI offers a comprehensive, coordinated program of both in-home care and Medical/Day Center care at the PACE site. Program participants receive a wide range of social, medical and health services that help them maximize their health and independence, and enable them to remain in the community. Without PACE, most participants could be living in nursing home facilities -- often far away from family and other community ties.
Services are managed through a care plan developed by an interdisciplinary team including the participant and the family. Services offered include rehabilitation, medical care, transportation interpreter services, social or recreational care, primary nursing facility care, medical specialty services, laboratory and x-ray services, ambulance service, and restorative or supportive services. CEI is licensed by the California Department of Health Services as a primary health care clinic, Adult Day Health Center, and home care agency.
